What I got from reading the Investigation report is that the force on the well pipe kinked it so it was jammed in the BOP at a diagonal and that at least one or two of the cutters actually engaged but the pipe was at the edge of the jaws (and not in the middle of the casing) and was not successfully cut through.

You would have to redesign the BOP to avoid this problem in the future.

I don't think so, those shear rams are designed to cut pipe (guessing you meaning drill pipe) from any angle. However there were 2 pieces of drill pipe sticking through the top of the well head after they cut off the drilling riser. The job of the annular rams is to grip the pipe, which would be gripped straight. Then the blind shear rams shear the pipe and seal the well bore and annulus.

The shear rams were not hooked up correctly and not maintained to specification as per Variable Bore Rams who manufactured the rams. I personally know the President of VBR. Was his rams on the Cameron BOP stack...

Think this had something to do with it, there should only be 1-piece of drill pipe going through that BOP. Drill pipe is extremely thick and heavy...

I remember seeing some shots of that at the time and thinking "what the hell are two pipes doing in the casing".

Since you seem up to speed, what the hell were two pipes doing in the casing?

Nobody knows for sure. Either a piece broke off under tremendous pressure and forced upward/unlikely or when the rig kicked a joint fell off rig floor down the riser and got lodged in the BOP. I remember seeing videos of it and there was only 1-joint of drill pipe spewing the oil and gas out. The other/nothing. It may have gotten stuck in the annulars preventing from gripping the other straight and causing the Blind Shear Rams to not work. I dunno. I've always said their had to be a chain of events both mechanical and physical for this to occur. If the dumb asses had done what they should have done according to protocol then the BOP would not have been needed....probably....
